The stems of Hop-Headed Barleria are green to slightly woody at the base, and they often develop small thorns along the branches. These stems support lush, glossy leaves that are oval to lance-shaped, about 6 cm to 12 cm long, and arranged oppositely along the stem. The deep green leaves have smooth edges and a prominent white or light pink midrib, giving the plant a healthy, vibrant look even when not in bloom.
